Electronic Request for Records

Abbeville General now offers an Online Patient Record Request tool that can be used any time, day, or night for the following individuals to request patient records:

§ Patients requesting their own records

§ Parents of minor patients requesting records

§ Caregivers acting on behalf of a patient (i.e. Power of Attorney)

The online tool features a modern design with clear, easy-to-use buttons and large simple text that’s easy to read and understand. It takes you step-by-step through each question, and your records can be securely delivered via email or by mail.

The online tool verifies your identity by asking you to upload or capture a photo of your driver’s license using a webcam or smartphone.

There is no additional charge to use this service. As a courtesy to our patients, there is no fee for copies of medical records for personal use. To help reduce processing time, patients are encouraged to request only the specific records they need rather than their entire medical record. 

Processing times vary depending on the type of records requested and the selected method of delivery.
