Electronic Request for Records
Abbeville General now offers an Online Patient Record Request tool that can be used any time, day, or night for the following individuals to request patient records:
§ Patients requesting their own records
§ Parents of minor patients requesting records
§ Caregivers acting on behalf of a patient (i.e. Power of Attorney)
The online tool features a modern design with clear, easy-to-use buttons and large simple text that’s easy to read and understand. It takes you step-by-step through each question, and your records can be securely delivered via email or by mail.
The Online tool verifies your identity by asking for a photo of your driver’s license, which can be taken via webcam or smartphone. There is no additional charge to use this service. However, please note that State and Federal laws permit fees to be charged for copies of medical records that are not requested by healthcare providers for direct patient care. To reduce costs incurred, patients should consider requesting specific information rather than a complete record. Processing time varies depending on the type of request and method of delivery.
